Our hours and appointment times vary depending on the provider. Please contact the individual provider to learn more about their hours, appointment availability, insurances accepted, and fees. You can contact our front office for guidance on getting started. If you are uncertain which therapist may be right for you, please read about each of our providers to see who may be the “best fit” for you! Our front office, as well as any of our providers, are here to help ensure that you are set-up with the right specialist for your therapeutic journey. Our Client Coordinator will help connect you with a therapist that meets your needs.

what should i expect on the first appointment?

Teleheath Appointments

Your therapist will send you a link to a secure, confidential portal that will allow your therapy session to occur online. Your therapist will meet you online at the scheduled appointment time. Together with your therapist, you will briefly review your documentation and you will be ready to begin on your therapeutic journey!

*It is extremely important that all documentation is completed prior to the first telehealth appointment. If the paperwork is incomplete, the appointment must be rescheduled.

In-Person Appointments

Your therapist will meet you in our waiting area. We ask that all documentation is completed prior to the first appointment. If you have any difficulties, your therapist will provide the documentation to you when you come to the office. Together with your therapist, you will briefly review the documentation and then you will be ready to begin on your therapeutic journey!

If you have any direct exposure to Covid-19, or any related symptoms, we ask that you reschedule your appointment.


what paperwork should i complete?

All necessary forms and information will be sent to you through the Client Portal, Therapy Notes. It is essential to complete these forms prior to your first appointment. If you are coming as a couple, each adult will complete the Adult Intake Form. For families, please complete an information form for each member of the family participating in therapy. Please complete the Release of Information form if there is another professional you would like your therapist to collaborate with for your most comprehensive care.

Can My Provider collaborate with others on my therapeutic or Educational Team?

Absolutely! On occasion, you may want your therapist to communicate with another professional to collaborate in your care. The Release of Information form below will allow you to give your therapist permission to do so. This form can also be shared with you directly through our Client Portal.

Do Your providers Accept insurance?

Most of our providers are out-of-network for insurance. We can provide you with the appropriate documentation to submit towards your out-of-network benefits. Once your benefits are met, your insurance company will reimburse you at their contracted rate. Coverage and reimbursement are determined by your contract with your insurance company. It is very helpful to be informed about your out-of-network benefits prior to your first appointment.
